PLEASE REMEMBER THE FOLLOWING ITEMS WITH YOU TO EACH APPOINTMENT!!!
- All Current Insurance Cards (a must at every appointment)
- All Current Prescription medication(s) our doctors prefer your bottles of medications, if you bring a medication list you must include the medication name, dosage (milligrams etc.) and how many times a day you take it.
- A list of vitamins, herbs, supplements, and over the counter medication(s) currently being taken. Again our doctors prefer to see the bottles.
- Previous medical records that may be helpful to your provider, especially if you have recently been in the hospital, seen in the Emergency Room or seen by a specialist or another Cardiologist. If you need assistance with this, please call our Medical Records Department at 575-521-3270 or 1-866-521-3270.
- Your completed Patient Forms. You will be asked to complete a Patient Registration form once a year to keep our records up to date, so please assist with this matter. It is necessary for billing purposes.
- As established by your insurance company, co-payments are due at the time of check-in. We accept cash, check, Visa and Master Card.
- Our office will attempt to get prior authorization from your Primary Care Physician or your Insurance Company for you. However it is ultimately your responsibility to make sure that there is a referral in place. In the event this information is not received we may cancel your appointment or request that you sign a waiver accepting responsibility for payment.
- You must bring a physicians order and/or referral to your visit, stating which tests or which provider he would like you to see.
New Mexico Cardiac Care, P.C. Also understands that it may be necessary to reschedule an appointment due to an unexpected circumstance. We ask that you please be considerate and reschedule 24 hours in advance to allow other patients to be seen at your assigned appointment time.
As new patient appointments are limited, it is our policy to consider dismissing patients from our practice that fail to give a courtesy 24 hour cancellation notice or fail to show after 3 visits for their appointments. Notifications are sent to you and the referring provider to make other arrangements for patient care.
